Dr. Peter Davenport specializes in providing comprehensive Class 1, 2 & 3 FAA flight physicals for pilots in the Four Corners area. As a certified FAA Aviation Medical Examiner (AME), Dr. Davenport is committed to providing timely and thorough medical examinations that comply with aviation medical standards.

FAA MedXpress
A MedXpress application is a MANDATORY pre-requisite to the Aviation Medical Exam (AME)
Complete an application in MedXPress:

Create an account in MedXPress

Once you have created an account, you will answer a series of medical questions and submit your application through MedXPress

After submission, print or save a copy of your application for Dr. Davenport (your AME). Dr. Davenport will need the confirmation number from the application in order to to perform your exam

You must complete the AME exam within 60 days of submitting the application, so schedule your appointment as soon as possible
Things To Expect In Your AME:

Verification of MedXPress confirmation number and government issued identification card

Use your full legal name that matches your other government IDs on MedXPress application to avoid delays

Review of any FAA letters or required documents

If you have an Authorization for Special Issuance (ASI), bring the authorization letter and all required reports to your exam

Review of Current and Past Medical History

For some conditions, the FAA may require medical information from your primary care physician

Review of Medications
-
Vital Signs (height, weight, pulse, blood pressure, etc.)

Physical Examination
-
Vision Test (visual acuity/depth perception/color vision)

Bring glasses or contacts
-
Color Vision Testing: Required for first-time applicants

Hearing test (audiogram)

EKG (if required)

Urine sample (drink fluids so you can provide a sample)
Please Note:

Without an FAA Application, we cannot perform your medical certification/recertification

You are responsible for/required to complete the FAA MedXpress Application each time you apply for medical certification/recertification

You MUST bring your printed summary or confirmation number to your appointment
IF YOU DO NOT BRING YOUR CONFIRMATION NUMBER, YOUR APPOINTMENT WILL BE CANCELLED AND YOU WILL BE CHARGED A $100 CANCELLATION FEE
Useful Information

Be sure to download LPFMAs app and complete all tasks prior to your appointment

All FAA paperwork must be completed through MedXPress prior to arrival

You must bring your MedXPress confirmation number and goverment-issued ID at the time of your appointment

Please arrive 20 minutes ahead of your appointment time

You will be required to leave a urine sample
-
Please notify us at least 24 hours in advance if an appointment needs to be cancelled, or a cancellation fee will be charged
