We are seeking a highly motivated LPN to provide clinical support to physicians. Our LPN will direct and monitor our COVID-19 vaccination program, screen patients for Annual wellness visits, assist with any and all office procedures, take phone calls and other clinical duties as assigned by management.

The Care Manager (CM) provides direct support to the physicians in managing the care of high risk patients. The CM will assist in the development of and be responsible for the implementation of care plans for these patients. The CM will be the primary point of contact for these patients and their caregivers in our office. The CM must communicate in a professional manner with our patients, our physicians, other medical offices, insurance companies and pharmaceutical companies.

Additional tasks include participation in quality improvement committees and team meetings, assist with the development of new clinical tools, forms and procedures, prepare and evaluate health outcome reports and/or clinical audits.
