Physician Partner - Family Medicine
La Plata Family Medicine is an independent, physician-owned primary care practice in Durango, Colorado seeking a Family Medicine Physician to join our team on a partner-track with a clear path to ownership.
We are not hospital-owned or corporate-managed. Our physicians practice relationship-based medicine with strong clinical autonomy, collaborative decision-making, and long-term operational stability.
This is a full-time outpatient position with a typical four-day clinical workweek, shared call, and no obstetric deliveries required.
The Opportunity:
· Partner-track opportunity with potential for ownership consideration after two years
· Independent primary care practice with established patient demand
· Four-day clinical workweek typical
· Shared call schedule (approximately 1 week every 6 weeks)
· No obstetric call
· Strong operational, clinical, and quality support teams
· Clinical autonomy within a collaborative practice environment
Responsibilities:
· Provide comprehensive outpatient family medicine care for patients of all ages
· Manage and grow an assigned patient panel
· Document timely and accurately in the EHR
· Collaborate with clinical and quality teams to support patient outcomes
· Participate in shared call and clinic operations
· Support a collaborative, team-oriented practice culture
Required Qualifications:
· MD or DO degree from an accredited medical school
· Board Certified or Board Eligible in Family Medicine
· Current Colorado medical license or eligibility for licensure
· Strong communication and documentation skills
· Commitment to relationship-based patient care

Quality Care Coordinator
We are seeking a Quality Care Coordinator (QCC) to support quality improvement initiatives, healthcare quality programs (e.g., Medicare’s Primary Care First, Aledade, Optum), and cross-functional duties within the practice. The role includes responsibilities for preauthorization, referral coordination, workers' compensation claims, and providing support to Front Desk Medical Receptionists as needed. The QCC ensures compliance with healthcare standards and assists in improving patient care outcomes. This position plays a vital role in facilitating timely, efficient care coordination across multiple healthcare providers and programs.
Medical Assistant - MA
We are seeking a Medical Assistant to provide clinical support to physicians. Medical Assistants will be expected to prepare patients for their visit by greeting them, escorting to exam room, providing exam clothing, gather and review medications, health history, and review of systems, as well as documenting vital signs. MA's will also assist providers with office procedures, provide injections, answer phone calls, complete admin work and any other duties as assigned by management.
